OBTAIN REPEAT PRESCRIPTIONS?

Repeat prescription slips are issued to patients who take regular medication and these should be posted with a stamped addressed envelope or given in at reception with items which are required marked with a tick.If you have more than one repeat prescription try to order all your items together. Please indicate if the prescription is to be picked up at reception or the local chemist by ticking the relevant section. All patients on repeat prescription should see their doctor at least once a year for a medication review. 48 hours' notice is required from the time the request has been deposited to the time when the medication is available for collection. To avoid errors telephone requests cannot be accepted. We may request you to make an appointment to see the doctor to review your medication from time to time.
